What should I bring to each appointment?
Current insurance card and photo ID.  These must be shown to the receptionist at EVERY visit
Appointment Reminder - will I get one?
There is an automated telephone reminder service in place. You should receive a phone call two business days before your scheduled appointment. The calls are made in the morning and the prompts will allow you to confirm your appointment. If you wish to cancel or reschedule your upcoming appointment please call the office during business hours (9:00 am – 5:00 pm M-Th, 9:00 – 4:30 pm Friday 773-871-4409 to do so. Occasionally the service will fail to call, but you are still responsible for keeping track of your appointment date and time. We also have a new text messaging reminder service in place. Texts are sent 2 days in advance of appointment between 5:00-7:00 pm. Text messages are an automated notification, and may not be replied to.
What is your appointment cancellation policy?
Appointments cancelled less than 24 hours in advance will be charged a cancellation fee of $25.00-$50.00. The fee will vary based on the length of the appointment. This charge is not covered by insurance. Please be considerate and give us as much notice as possible if you need to change or cancel an appointment, in order to free up your slot for another patient who may need it. New patients who cancel less than 24 hours in advance will be charged $100.00.
What if I forget my insurance card ?
You will be asked to fax, email or mail a copy of your insurance card to us within 24 hours of your appointment. In the meantime, if we do not have verified insurance information for you on file we will consider you a self-pay patient and will need to charge your credit card for the visit. This charge can be reversed if we receive and are able to verify your insurance information within 2 business days. ***A copy of your insurance card front and back may be scanned and emailed to insurance@pcmachicago.com. This is not a secure email address, however it is an option if you choose to provide your information this way.
Can I email my physician?
If you are signed up for our Patient Portal you will be able to securely send a message to your physician’s care team.  Either the physician or a member of the care team will view the message.  You can expect a reply within 2-3 business days.  These messages are not meant to replace an office visit and are not meant for questions that require an immediate answer.  If you need a response sooner than 2-3 business days, please call the office 773-871-4409.  You may be charged for medical advice given through the Patient Portal.
What is the Patient Portal?
The Patient Portal is our secure website that allows existing registered patients to request appointments, request prescription refills, view recent labs, pay bills online, and securely send a message to your physician’s care team. Ask our receptionists for your PIN number at your next office visit so that you can sign up for this free service. New patients can register for the portal following their first appointment and receipt of their PIN number.
Are the physicians accepting new patients?
Yes, all of our providers are accepting new patients at this time, but may not always be able to schedule a new patient appointment in a timely fashion. Please be aware that you may need to schedule a new appointment several weeks in advance. We appreciate your patience during this process.
New Billing Policy
We established a new financial policy in 2015 to clarify our policies and procedures. You will be given an agreement to sign upon your next visit to the office. You may also download it under the Patient Resources Tab-Forms.
